How to Submit an Incident Report through Web Dashboard

① Log in to the SkillSignal Admin Center and go to "Incident Reports" module.

② Select Project on the dropdown.

③ Click the "+ New Incident Report" button.

④ Select the category to report an incident and hit "Next".

⑤ Whether it's "Injury or Illness" Incident Report or "Near Miss", complete the necessary fields by selecting the "OPEN ALL" button.

  • Injury Locations (Front)

⑥ Once all the parts/fields are completed, hit "Submit for Verification".
